You must submit copies of the forms PRIOR to the start of the games. Each athlete needs a valid medical and consent form on-site to participate. REMINDERS | | Medical and Consent Binder
The Head Coach of a team/training club must present their Med/Consent binder at the beginning of each event and it should also be present at all training sessions/trips etc. This will be checked at registration on the day of the event and should include valid Meds/Consents for each of the athletes participating. If you do not bring your binder to the event we cannot allow your team/individual to participate. Please contact Colleen Everett at ceverett@nyso.org or (212) 661-3963 as soon as possible if you are requesting any copies from the NYC Region office for the 2008 Metro Tournament or Summer Games.
